About Care Connect Navigators

Our Story

We started this journey because we care deeply. We’ve seen first-hand how the right professional tailored NDIS supports can change a person’s life, and we believe everyone deserves that chance. For us, it’s not just about delivering services, it’s about walking alongside people as they build confidence, find their voice, and live life on their terms.
We believe that every person — no matter their ability — deserves to feel safe, respected, and truly seen. We’re here for the moments that matter: the first steps toward independence, the small wins that build self-belief, and the everyday achievements that others might overlook but we celebrate fully.


We do this because we want to be part of something meaningful. Because people deserve more than just a provider — they deserve a team that listens, shows up, and cares as much about their goals as they do.

Our Vision

We want people with disability to live with independence, dignity, and purpose. We believe everyone should have real choice and control over their life and supports. Our goal is to be a trusted local provider in Cairns and Far North Queensland, known for reliable support and positive outcomes for participants. 

Our Mission

We work alongside participants, families, and referrers to help people understand and navigate the NDIS, build skills, and access the right supports. We provide consistent, person-centred services that are shaped around each person’s goals and needs. We understand the challenges of living in regional and remote areas and are committed to supporting people where services and choices may be limited. Every day, we show up with care, honesty, and purpose so people feel listened to, respected, and supported.
